Referrals

Patients are typically referred for hospice care by their personal physicians or by other healthcare professionals. However, anyone such as family members, friends, a member of the clergy, or patients, may request hospice care.

To Receive Hospice Services:

1. Patients are referred to hospice by their physicians after a confirmed diagnosis of terminal illness.
2. Patients and their families have decided to receive comfort measures for their illnesses rather than a cure.

Legacy Hospice uses a coordinated program of support services for pain and symptom control for terminally ill people and their families. Our program provides home care services with arrangements for inpatient care as needed.

The following may be classified as terminal illnesses:

  • End Stage Parkinson’s
  • Cancer
  • End Stage Alzheimer’s
  • End Stage CHF (Congestive Heart Failure)
  • End Stage COPD
  • Lymphoma
  • HIV (End Stage)
  • Leukemia
  • End Stage Renal Failure
  • Paget’s Disease
  • ALS
  • Recurrent Aspiration Pneumonia
  • Other illnesses with a terminal diagnosis of six months or less.

Who Pays for Hospice Care?

Reimbursement for services may come from:

  • Medicare
  • Medicaid
  • Private Insurance
  • Other

This facility complies fully with Title VI of the Civil Rights Act of 1964, Section 504 of the Rehabilitation Act of 1973, Age Discrimination Act of 1975, and the HIPPA Privacy Act.
